When someone needs long-term care in a care home or nursing home, one of the most important tools to ensure that it is person-centred is the care plan.

A personal care plan tells our staff at Park Vista about the resident. It covers important information about the resident, and their personal and medical needs.

  • About the person. Such as likes and dislikes, hobbies, ...
  • Personal needs. Assistance required such as help with dressing, ...
  • Medical needs. Medicines prescribed, medical conditions, how they are managed, ...

Preparing the Plan

We start to prepare the Plan at the assessment meeting with the prospective resident and their family.

Further medical information is gathered from their doctor or hospital records if coming to us from hospital.

When you move in we will spend time with you and your family to gather more information about you, and to assess your needs and how to meet them.

Providing Care

The Personal Care Plan is regularly reviewed by staff at Park Vista with the resident and their family to ensure it is kept up-to-date and relevant.
